- I am a social worker or other child advocate. Can I submit an online application on behalf of the family?
- The child's parent or legal guardian must submit the online application.
- One of our patients is requesting a doctor's (M.D. or D.O.) letter to be sent to your organization. What is this for?
- The Foundation requires this information so the Board can understand the medical condition and if the doctor supports what the family is asking for in the grant application. The doctor's opinion is important to us because we want our potential grant recipients to receive the care their doctor recommends.
- On this Web site there are examples of the types of medical services covered by UHCCF medical grants, such as speech therapy, physical therapy, wheelchairs, hearing aids, etc. What if the request is not one of these examples? Are there certain items that are excluded?
- The Foundation excludes several items from grant consideration. Please go to the applicant page and click on the application criteria link.
- Despite the current exclusions referenced above, please keep in mind the Board of Directors pays close attention to the efficacy of the medical treatment being requested.
- Applications must be medical in nature.
- Do you make medical grants to other charities, organizations or businesses?
- No. We focus exclusively on individual children and their families on a case by case basis.
- Can you send us brochures to hand out?

- One of our patients applied for a grant from the Foundation. How do we determine the status?
- The Foundation does not communicate with third parties about the status of grant applicants. Please have the family communicate with us directly.
- One of our patients has an approved grant from the Foundation. How do we determine the status of a grant payment?
- The Foundation does not communicate with third parties about the status of grant recipients and grant payments. Please have the family communicate with us directly.
- We have families without a computer who would like to apply. Do you have a paper application?
- Please encourage the family to go to their public library and use their free internet service to apply online.
- One of our patients has an approved grant from the Foundation. The family is asking for detailed invoice information. What are you looking for?
- If a grant is approved for a child, the Foundation will pay for approved services/items after the family's commercial health insurance submits payment, if any. In order to pay for approved services/items after insurance, the Foundation requires very clear and detailed invoices/bills. We require your tax identification number, contact information, dates of service, description of the service, cost, what insurance paid (if any) and the total amount due. In the event that your patient must pay at time of service (e.g. co-pay), please provide them with a receipt, plus a detailed invoice/bill referenced above.
